Health Forms for School, Camp and Sports
To offset the expense of the data entry and file maintenance, we charge a $5.00 fee per form. We encourage you to make copies of this form for other uses. The cost for handling medical records is not covered by commercial insurances or health maintenance organizations. Alternatively, if parents wish to complete their own forms and maintain their own shot and health records for their children, we will complete the "physician's" evaluation section of those forms at no charge.

The
number of health forms required for your children from school, camp
and sports programs is amazing. The information required on these
forms is usually repetitious. In order to meet the need to supply
medical history and exam forms for various organizations, we developed
a standardized form which we maintain on our office computers.
